HIV Risk among Men Who Have Sex With Men Who Have Experienced Childhood Sexual Abuse: Systematic Review and Meta-Analysis  / Shane Lloyd, Don Operario.

samenvatting: Previous research has indicated a high prevalence of childhood sexual abuse (CSA) among men who have sex with men (MSM) in the United States, and has suggested that CSA history is a risk factor for HIV infection in MSM. We conducted a systematic review to identify, synthesize, meta-analyze, and critique the current state of relevant literature. Systematic review methodology was utilized to identify 12 studies that compared MSM with a history of CSA to MSM without a history of CSA on HIV risk indicators including HIV serostatus, sexually transmitted infections (STIs), sexual behaviors, and illicit drug use. Overall, 27.3% (n = 4,263) of the MSM in all included studies (n = 15,622) reported a CSA history. Across the studies that used probabilistic sampling (n = 8,240), the estimated prevalence of CSA was 21.8% (n = 1,800). Meta-analysis indicated that MSM with CSA history were more likely to be HIV positive [odds ratio (OR) = 1.54; 95% confidence interval (CI) = 1.22?1.95)] and to engage in recent unprotected anal intercourse (OR = 1.85, 95% CI = 1.36?2.51). Studies also indicated that MSM with a history of CSA were more likely to report frequent casual male partners, substance use, and sex while under the influence of alcohol or other drugs. Trends across studies indicated a need for interventions to assess CSA history and address effects of CSA on sexual risk behavior of MSM. Inconsistencies across studies indicated a need to reach consensus among researchers and providers in defining CSA.

samenvatting: Forging new concepts 'kid Orientalism, reverse pedophilia, and manifest latency 'this essay speculates on something that's been surfacing in Anglo-American public culture over the last ten years or so. A future the public fears is coming - child sexuality, evidenced by sexting, 'gay' kids in middle school, and sexual bullying - is accompanying exportation of a fading child (the figure of the innocent child) to other lands, where it seems weirdly possible to recover it. Quite paradoxically, the aesthetics of world documentaries on the-child-in-peril-in-the-third-world may be 'restoring' the 'Western'-style innocent child through, of all things, the sexualized, racialized 'HIV child'. Yet this child-in-peril becomes a threat in a different direction, because of the demands it makes for our response. Feeling thus threatened, we flee from children who, we imagine, are desiring us (in reverse pedophilia). Is there any antidote to these strange dynamics? How does experimental literary form in the novel Push, and something Stockton calls 'lyrical fat' in the film Precious, work against this fray? Stockton finds answers among depictions of children's passion for signification?children's libidinal relationships to signifiers?and through new conceptions of sexual latency via the latency of signification. samenvatting: This essay examines the production of Native queer childhood in nineteenth- and twentieth-century boarding schools. I argue that the schools produced 'sexual orphanings' in Native children, which was achieved through education and abuse that queered Native children's conceptualizations of gender, sexuality, and kinship and produced shame. Sexual orphanings queered children away from their bodies and sexualities, which also worked to orient them away from the future. I turn to Tomson Highway's Kiss of the Fur Queen (1998) to consider what erotics might remain for these children and to look at how genocidal sexual orphanings might be recalibrated in Native fiction toward survivance rather than disappearance. samenvatting: Sexual-minority women (SMW) are more likely to be overweight, have a history of childhood sexual abuse (CSA), and have more positive attitudes toward obesity compared to their heterosexual counterparts. In this exploratory study, the author sought to identify any associations between CSA, obesity, and attitudes toward obesity among SMW. Eighty-five SMW completed a survey assessing their weight, history of CSA, and attitudes toward obesity. An ordered logistic regression (OLS) and an analysis of variance (ANOVA) revealed CSA to be negatively correlated with BMI, as well as no association between SMW's attitudes toward obesity and CSA nor BMI. Lastly, SMW did not report significantly different attitudes toward BMI based on their own BMI and history of CSA. This study provides evidence that SMW, particularly those residing within the Deep South, are at high risk for both CSA and for obesity, and highlights the importance of developing future culturally tailored weight loss programs designed specifically for SMW with histories of trauma.

samenvatting: Little research has compared the mental health and victimization experiences of non-binary youth depending on their sex assigned at birth (SAAB), or compared these two groups with binary transgender youth. Aims: To compare mental health, self-harm and suicidality, substance use and victimization experiences between non-binary and binary transgender young adults, both male assigned at birth (MAAB) and female assigned at birth (FAAB). Methods: Online survey data from 677 participants from the 'Youth Chances' community study of 16 to 25 year olds in the United Kingdom was analyzed, comparing across binary participants (transgender females (n = 105) and transgender males (n = 210)) and non-binary participants (MAAB (n = 93) and FAAB (n = 269)). Results: Female SAAB participants (binary and non-binary) were more likely to report a current mental health condition and history of self-harm than male SAAB participants (binary and non-binary). Similarly, female SAAB participants (binary and non-binary) were more likely to report childhood sexual abuse than male SAAB participants (binary and non-binary); the reverse pattern was found for lifetime physical assault relating to being LGBTQ. Non-binary MAAB participants were less likely than the other groups to report past suicide attempts and previous help-seeking for depression/anxiety. Binary participants reported lower life satisfaction than non-binary participants. For all four groups, mental health problems, self-harm, suicidality, alcohol use and victimization experiences were generally higher than that of youth in general population studies. Conclusions: These findings highlight the importance of considering both non-binary versus binary gender identity and SAAB in relation to mental health problems, self-harm, suicidality and substance use in transgender youth. The roles of sexual abuse, other abuse and discrimination in contributing to increased rates of mental illness and self-harm in non-binary and binary transgender individuals, particularly those who were assigned female at birth, relative to those assigned male, require investigation.

samenvatting: The effects of sexual abuse in childhood are serious and far-reaching, and include a number of short- and long-term sequelae, including depression, anxiety, disordered eating, post-traumatic stress disorder (PTSD), substance abuse disorders, suicidal ideation and self-injurious behavior, sexual dysfunction, borderline personality traits, high-risk sexual behavior, and dissociative disorders. MSM (men who have sex with men) with a history of childhood sexual abuse are more likely to engage in high-risk sexual behavior, trade sex for substances and/or money, experience intimate partner violence, and report as being HIV-positive. In this case study, we aim to explore the association between sexual abuse in childhood and HIV/AIDS. We identify a HIV-positive gay man with a history of sexual abuse and review the existing literature on the subject of HIV and sexual abuse, with a focus on victims who are MSM. Early childhood traumas, especially sexual abuse, can have immense and long-lasting psychological sequelae. Here, we report a victim of childhood sexual abuse who suffers from PTSD, selective amnesia, and episodes of dissociation during sex who was diagnosed as being HIV-positive at the height of the HIV/AIDS epidemic in the United States. More research needs to be done to investigate the complex relationship between childhood trauma and HIV infection, with insights into racial, gender, and socioeconomic differences. Counseling and risk-reduction strategies need to be explored with patients with a history of sexual trauma, in tandem with long-term pharmacologic and therapeutic interventions.
